Click the 3-dot action menu of the Option grant that you wish to cancel and click 'Cancel'

Input the cancellation details into the window:

Cancellation Date - Input the date the cancellation occurred (This cannot be in the future and must be after the most recent approved exercise date). This is a required field.
Reason for cancellation - select a reason for cancellation from the dropdown menu. This is a required field.
Comments - if you wish to enter a comment regarding the cancellation you can input them here. This is not a required field.

Once you have entered the details in the window select 'Submit'
