Column	Header Name	Description
C	Employee first name	The Grant Holders First Name as populated 'Grant' screen
D	Employee second name	The Grant Holders Second (or middle name as per the platform) If no middle name is populated the this field will remain blank, as populated on the 'Grant' Screen
E	Employee last name	The Grant Holders Last Name as populated on the 'Grant' screen
F	National Insurance Number	The Grant Holders Tax ID as populated in the 'Users' Screen
G	Number of shares over which the option was granted	The total number of EMI Options the Grant Holder was awarded. As populated in the 'Grant Screen'
H	Exercise price per share £	This is the 'Grant Price' that is populated on the grant screen and is the Price per Share that the Grant Holder will pay for the shares.
I	Actual market value per share £	This is populated if AMV is recorded against this grant on the grant screen (It is only possible to add AMV Valuations for companies with a country of incorporation in the United Kingdom. AMV valuations can be added by navigating to the 'Company Overview' screen, clicking on 'Valuations' and clicking 'Add Valuation'). AMV valuations can be added to new EMI option grants by attaching them at the time of award, or by editing an existing EMI option using the 3 dots action menu on the grant screen.
J	Total unrestricted market value at date of grant of employee's unexercised options including CSOP options £	This is not populated by Global Shares and needs to be manually inputted once the template has been downloaded. If the employee has any unexercised CSOP options as well as their EMI options, enter the total unrestricted market value of both sets of unexercised options There is a £250,000 limit on holding options over shares. Any options you grant the employee over this will not be qualifying options. When this happens you must register a non-tax advantaged scheme
